I have joined this forum just to reply to this problem. I have had the same trouble with my 2002 Grand Voyager 2.5 CRD. First would not start after a short journey, then started after 20 mins break. Then started cutting out once she was warm, would sometimes restart straight away but mostly had to leave it for 20 to 30 mins.
